Due to the small dose level of neurotoxins used, serious adverse effects are rare—five reported incidents per million doses in one given year—when Botox® is injected for cosmetic purposes. And there are no known long-term or life-threatening adverse effects related to Botox® cosmetic use. Serious adverse effects are more common when Botox® is used for therapeutic purposes.

Transient and benign side effects—bruising, localized pain, headache, and skin reddening—are more common with cosmetic Botox® injections. However, these side effects are typically localized, reversible, and usually disappear independently without additional treatment.
